In an innovative and exciting project OwnHealth Birmingham, a partnership between North and Eastern Birmingham PCTs, UK Pfizer Health Solutions and Our Client, will provide a telephone based care management programme to support people with long term medical conditions take a more active and informed role in managing their own health.
We are looking for talented nurses who will support their own caseload of patients to become confident and empowered in the management of their conditions (Diabetes, Coronary Heart Disease and Congestive Heart Failure).
You will promote wellness, improved outcomes and self care within a defined population of patients with a long term condition(s) and support appropriate utilisation of health services. Your role will involve working with the individual patient, GPs and other providers of care to develop and implement individual health plans and provide support to the patient in self management.
With your help these patients will live fuller, longer and more healthy lives.
You will preferably have experience of working in the field of chronic disease management but will definitely enjoy coaching and supporting patients. You will be based at our call centre in Brierley Hill. Hours of work are flexible (minimum of 20 hours / week).
Ideal Qualifications/Skills:
* First level NMC Registered Nurse
* Preferably, Clinical skills in diabetes/CHD/Heart Failure
* At least 3 years post registration
We are recruiting for a new team and full training in Care Management and telephone support will be given. Urdu, Punjabi or Somali? speakers are particularly encouraged to apply, to provide care to the diverse population of the area.
