Senior Social Work Practitioner
Specialism: Mental Health
Salary: £33,000 - £36,000
Role Summary:
The overall aim of our client?s social care department is to work collaboratively with health professionals and other agencies to ensure the needs of individuals are addressed in a manner which strives to empower people, to aid their recovery and support discharge into an environment designed to promote their health and well being.
Main Responsibilities:
? Provide day to day supervision for the social work team and monthly clinical supervision for social work practitioners.
? Complete comprehensive social work assessments as required from the pre admission to discharge stage incorporating individual need and risk assessment/management.
? As part of the multidisciplinary team, take responsibility for liaison with originating authorities (Health, Social Services Departments and Probation Services) pre or at admission through to discharge stage.
? Offer a wide range of social work interventions to individuals, families and carers ensuring interventions and practices which reflect anti-discriminatory practice.
? Ensure individuals and their families are involved in the decision making process regarding their care wherever possible.
? Maintain appropriate records of work undertaken and complete the required administrative procedures.
? Prepare reports as required for Mental Health Review Tribunals and appeals to Hospital Managers. Attend hearings as required.
? Liaise with the appropriate Local Authority and other relevant social care agencies to gather and share information relevant to the welfare and protection of children.
? Undertake the role of appropriate adult under P.A.C.E. 1984.
? Undertake practice teacher training and act as practice teacher for social work students.
? Participate in the training of social workers and other professionals.
? Attend supervisory, team meetings and staff meetings and undertake professional training and development as appropriate.
? To abide by the communication policy of the service and to develop own sign language skills to CACDP level II.
